License Requirements

  1. Valid Quittance Certificate from Municipality attached to the application.
  2. A copy of the Applicant's I.D. Card.
  3. The applicant must bring the document that prove the status of getting the certificate.
  4. A letter from the authority requesting the certificate.
  5. Issuing a map signed from the One Stop Shop and the Engineering Department.
  6. Crafts License must be conditioned in the duration required for practicing the profession (Profession Practicing Certificate).
  7. Applicant's signature on the attached form.
  8. Certificate of Quittance from the Department of Property Taxation.
  9. Accredited form from the Department of Property Taxation- Possession document of property or land.
